Ouachita Parish Police Jury (hereinafter referred to as the “Owner”)
Sealed bids marked “Sealed Bid – Ouachita Parish Police Jury Disaster Recovery Unit Project No. 37PARA3401 to be financed by the State of Louisiana DRU Program” will be received by the Owner for the construction of the project described as follows:

Proposals shall be addressed to the Ouachita Parish Police Jury, and delivered to the office of the Recording Secretary located at 301 South Grand Street, Room # 201, Monroe, Louisiana 71201 not later than 2:00 o’clock P.M. CDT, on the 3rd day of January, 2013. Sealed bids to be marked “Sealed Bid – Ouachita Parish Police Jury Disaster Recovery Unit Project No. 37PARA3401 to be financed by the State of Louisiana DRU Program”. All bids must be submitted on the Bid Forms provided in the Specifications and Contract Documents. Any bids received after the specified time and date will not be considered. The sealed bids will be publicly opened and read aloud at 2:00 o’clock P.M. on the 3rd day of January, 2013, in the Police Jury Conference Room in the Ouachita Parish Courthouse located at 301 South Grand Street, Monroe, LA. The Information for Bidders, Form of Bid Proposal, Form of Contract, Plans, Specifications, and Forms of Bid Bond, Performance Bond and Payment Bond, and other contract documents may be examined at the Office of Lazenby & Associates, Inc., 2000 North 7th Street, West Monroe, Louisiana 71291 (Phone (318) 387-2710). Copies may be obtained at this office upon payment of $50.00 which amount constitutes the cost of reproduction and handling. This payment will not be refunded.
The Owner reserves the right to waive any informalities or to reject any or all bids, except the waiver of signing of: 1) the bid; 2) execution of the certificate of corporate principal and surety; 3) acknowledgement of any addenda issued; 4) bid bond which always must accompany the bid in the correct amount; and 5) the requirement of a corporate resolution if someone other than a corporate officer signs for the bidder/contractor. Such activities will be in accordance with Title 38 of the Louisiana Revised Statutes.
Each Bidder must deposit with his/her bid, security in the amount, form, and subject to the conditions provided in the Information for Bidders. Sureties used for obtaining bonds must appear as acceptable on the U.S. Department of Treasury Circular 570.
No bidder may withdraw his/her bid within forty-five (45) days after the actual date of the opening thereof.
The Attention to Bidders is called particularly to the requirements for conditions of employment to be observed and minimum wage rates to be paid under the Contract, Section 3, Segregated Facilities, Section 109, Executive Order 11246, and all applicable laws and regulations of the Federal government and State of Louisiana and bonding and insurance requirements.
For contractor information, this project is classified as Highway, Street, & Bridge Construction.
The successful bidder must submit executed copies of certification regarding Equal Employment Opportunity, Section 3 and Segregated Facilities, Section 3 Plan with Tables A & B, Certification Concerning Labor Standards and all subcontractor’s certifications, Non Collusion Declaration, Attestation Clause, and E-Verify Affidavit prior to contract award.
The successful bidder will be required to execute the Owner’s Standard Form of Agreement/Contract for construction together with the Performance and Payment Bonds within ten (10) working days after normal award of contract.

NOTICE
Proceedings of the Board of Commissioners of the West Ouachita Sewerage District No. 5 Ouachita Parish, Louisiana. Taken at the Office of the District, 327 Wallace Road, West Monroe, Louisiana. Monday-November 19, 2012. (Regular meeting)
Members present: Fred Hall, Ralph Owens, Don Leach
Members absent: None
Others present: David Soignier - Luffy, Huffman, Radsdale & Soignier, APAC
Jerry Lazenby - Lazenby a& Associates, Inc.
Terry Cox - District Manager
The first item on the agenda was the financial statements for the three months and year ended August 31, 2012. Mr. Soignier presented the statements to all present and following a brief discussion all agreed the district is in sound financial condition. No action was taken as nine was necessary/
The next item was the two (2) duplexes on Caldwell Road by contractor Dean Roy. Mr. Lazenby presented the plans and specifications and referred to a letter written on November 1, 2012 (copy given to Mr Roy on November 5, 2012) concerning the elevation of the property. Following a brief review Commissioner Owens made a motion to accept the duplexes and Commissioner Leach seconded the motion. The motion passed by a unanimous voice vote.
Next was the final change order for the lift station "N' Force Main Project. Mr. Lazenby presented the order and following a discussion Commissioner Leach made a motion to accept the change order and Commissioner Owens seconded the motion. THe motion passed by a unanimous voice vote.
The Flagstone Subdivision on Standard Reed Road was the next item. Mr. Lazenby brought everyone put to speed on the progression of the subdivision and no action was taken as none was necessary.
Mr. Lazenby presented the plans and specifications for a new CVS Pharmacy on the corner of HWY. 80 and Wallace Dean Road. A brief discussion followed and Commissioner Owens made a motion to accept the pharmacy and Commissioner Leach seconded the motion.The motion passed by a unanimous voice vote.
The final item was the cash disbursements. Mr. Cox presented the disbursements for the month and a brief discussion followed. Commissioner Owens made a motion to accept them as reasonable and necessary and Commissioner Leach seconded the motion. The motion passed by a unanimous voice vote.
No further business being presented the meeting adjourned at 12:10.

Commissioner Hall called the meeting to order promptly at 111:00 a.m. o'clock.
The first item was a request from Greater Ouachita Water Company, for an extension of six months, on the Backwash Abatement Agreement. Lr. Lazenby presented the request to the board members, and a discussion followed. Commissioner Leach made a motion to extend the abatement and Commissioner Owens seconded the motion.
The motion passed by a unanimous voice vote.
Next, Mr. Lazenby brought everyone up to speed on the profess of lift station "N" Force Main Project. No action was taken as none was needed.
The final item was cash disbursements. Mr. Cox presented the disbursements of the month and a discussion followed. Commissioner Owens made a motion to accept the disbursements as reasonable and necessary and Commissioner Leach seconded the motion. The motion passed by a unanimous voice vote.
No further business being presented but meeting adjourned at 11:45 a.m. o'clock.
