PERFORMANCE HEARING / PUBLIC NOTICE #3
The City of West Monroe will hold a Public Hearing to obtain citizens' views and to respond to questions relative to the Town's performance on its FY-2011 LCDBG Street Project # 708721. The meeting will be on Friday, October 5, 2012 @ 11:00 A.M. in the City of West Monroe’s City Hall.
The Louisiana Community Development Block Grant Program (LCDBG) has established four (4) major goals (objectives) for Community Development Programs:
A. Strengthen community economic development through the creation of jobs, stimulation of proven investment, and community revitalization, principally for low and moderate-income persons.
B. Benefit low-moderate income persons.
C. Eliminate or aid in prevention of slums or blight.
D. Provide for other community development needs having a particular urgency because existing conditions pose a serious and immediate threat to the health or welfare of the community where other financial resources are not available to meet such needs.
This Public Hearing will include a discussion of Town’s performance on their FY-2011 LCDBG Street Project; the amount of funds expended, the benefit to low/moderate income persons, and related Project outcomes. Project records are on file at the Town Hall and may be reviewed by citizens each working day during regular working hours (8:00 a.m. - 5:00 p.m.).
All residents of the City of West Monroe are invited to attend this Hearing and to review Project Performance and to express their views and make comments both written and verbal on all aspects of the program and the Project. Written comments can be sent to Mayor Dave Norris, 2305 N. Seventh Street, West Monroe, LA 71291. Facilities for the handicapped will be provided, as will interpreters for non-English speaking persons. A regular meeting of the Board of Commissioners, Hospital Service District No. 1 of the Parish of Ouachita, State of Louisiana was called to order at 11:35 A.M. at the Wellness Center, 3215 Cypress Street, West Monroe, Louisiana, by Steve Hall, Chairman of the Board of Commissioners. All references to the “District” refer to the Hospital Service District No. 1 of the Parish of Ouachita, State of Louisiana. All references to the “Foundation” refer to the Living Well Foundation.

REMARKS FROM THE CHAIRMAN OF THE DISTRICT
Mr. Hall stated that the fire marshal has approved the plans for the expansion of the Wellness Center and the permit is forthcoming. The project should begin the week of August 27, 2012.

APPROVAL OF MINUTES OF DISTRICT BOARD MEETING OF JULY 24, 2012
After review of the minutes of the District’s July 24, 2012 Board meeting, a motion was made by Mr. Spencer and seconded by Mr. Mulhearn to adopt the minutes as presented and the motion was unanimously approved.
REVIEW OF SECOND ANNUAL REPORT FROM THE OUACHITA PARISH SCHOOL BOARD UNDER THE HSD-OPSB COOPERATIVE ENDEAVOR AGREEMENT
Ms. Robin Austin, the OPSB Grant Facilitator, and Linda Trimble, Grant Coordinator, were in attendance to present the second annual report under the HSD-OPSB Cooperative Endeavor Agreement. Ms. Austin reported that since their last annual report, very positive feed back has been received regarding some recent workshops and personnel are becoming more receptive. Plans are being made for walking tracks at some of the schools. Per the District’s attorney, the written report appears to be in compliance.
EXECUTIVE SESSION FOR PURPOSES OF STRATEGIC PLANNING, INCLUDING WITH RESPECT TO THE FUTURE OF THE WELLNESS CENTER AND RELATIONS WITH GRMC, GLENWOOD RESOLUTION AUTHORITY, INC., AND THE LIVING WELL FOUNDATION
A motion was made by Mr. Spencer, seconded by Mr. Gilley, and unanimously approved for the Board of Commissioners to go into executive session for the purposes of strategic planning, including reviewing the Wellness Center monthly financial statements.
Mrs. Culp, Executive Director of the Wellness Center, presented the Wellness Center monthly financial statements and her management report.
Mr. Mulhearn motioned, Mr. Gilley seconded, and the Commissioners present approved the motion to go back into public session.
REVIEW OF CURRENT FINANCIALS OF DISTRICT (OTHER THAN THE WELLNESS CENTER)
Mr. Hall presented the monthly financials of the District (other than the Wellness Center) ending July 31, 2012.
RECOMMENDATION TO OUACHITA PARISH POLICE JURY OF COMMISSIONER TO BE APPOINTED TO REPLACE VIOLET LINER
The Commissioners continue to work toward filling the vacant unexpired term of Violet Liner.
ANNUAL REVIEW OF INSURANCE OF THE DISTRICT
An annual review was made of the District’s insurance as prepared by Phillip Sivils, the District’s insurance consultant.
ANNUAL REVIEW OF COORDINATION AGREEMENT WITH THE OUACHITA PARISH POLICE JURY
The Commissioners reviewed the Coordination Agreement with the Ouachita Parish Policy Jury, noting the importance of continued compliance. Mr. Jay Mitchell, attorney for the Ouachita Parish Police Jury, was in attendance to discuss the possibilities for future involvement with the Picard Center for Child Development and Lifelong Learning. Future meetings are planned with the Living Well Foundation and there could be a need for an amendment to the agreement regarding the School-Based Health Clinics.
ANNUAL REVIEW OF COOPERATIVE ENDEAVOR AGREEMENT WITH LIVING WELL FOUNDATION
The Board discussed the stipulations of the Cooperative Endeavor Agreement with the Foundation, noting that all appeared to be in compliance, with the possible exception of some dysfunctional links on the website for the Foundation’s minutes and no appearance of the most recent annual audited financial statements, Form 990, and Annual Report of the Foundation. Mr. Landry will discuss these issues with the Foundation’s attorney.
QUARTERLY RECEIPT/REVIEW OF LIVING WELL FOUNDATION’S UNAUDITED QUARTERLY FINANCIAL STATEMENTS
The Foundation’s Unaudited Quarterly Financial Statements for June 30, 2012 were presented for review and discussion.
REVIEW OF THE FOUNDATION’S GRANT ANNOUNCEMENT FOR FALL 2012 GRANT CYCLE
The Foundation’s grant announcement for the fall 2012 grant cycle was discussed and no questions or concerns were presented.
REVIEW OF LIVING WELL FOUNDATION’S EXECUTIVE COMMITTEE MINUTES OF JULY 10, 2012 AND AUGUST 7, 2012
The Board of Commissioners reviewed the Foundation’s minutes of the July 10, 2012 and August 7, 2012 Executive Committee meeting.

REQUEST FOR RELEASE OF FUNDS
On or about October 15, 2012 the Ouachita Parish Police Jury will submit a request to the Office of Community Development (OCD) for the release of funds under the FY 2008 Consolidated Security Disaster Assistance and Continuing Appropriation Act, 2009 (PL 110-329), in accordance with the “Notice of Funding Availability” published in the Ouachita Citizen on September 27, 2012, for the purpose of installing approximately 570’ of 24” storm drainage pipe will be installed along the south side of Wellerman Road from Italiana Place to Janna Drive for the City of West Monroe. This new pipe will collect runoff from Janna Drive to Italiana Place. Approximately 900’ of new 42” pipe will be installed from Janna Drive to Warren Drive. This new 42” pipe will tie into subsurface drainage along Warren Drive. This project also involves installing approximately 570’ of new pipe behind the residences along White Columns Drive. This pipe will also tie into the subsurface drainage along Warren Drive. These new drainage structures will alleviate the flooding problems in the target area. The amount of CDBG Disaster funding requested for this project is $593,163.00
FINDING OF NO SIGNIFICANT IMPACT
The Ouachita Parish Police Jury has determined that the project will have no significant impact on the human environment. Therefore, an Environmental Impact Statement under the National Environmental Policy Act of 1969 (NEPA) is not required. Additional project information is contained in the Environmental Review Record (ERR) on file at the Ouachita Parish Police Jury, 300 Saint John Street, Monroe, LA 71201, (318) 327-1340 and may be examined or copied weekdays 9 A.M. to 2 P.M.
PUBLIC COMMENTS
Any individual, group, or agency disagreeing with this determination or wishing to comment on the project may submit written comments to the Ouachita Parish Police Jury, Post Office Box 3007, Monroe, LA 71210-3007. All comments received by October 12, 2012 will be considered by the Parish prior to authorizing submission of a request for release of funds. Commentors should specifiy which part of this Notice they are addressing.
RELEASE OF FUNDS
The Parish certifies to the Office of Community Development (OCD) that Shane Smiley in his official capacity as President consents to accept the jurisdiction of the Federal Courts if an action is brought to enforce responsibilities in relation to environmental review process and that these responsibilities have been satisfied. The Office of Community Development (OCD) approval of the certifications satisfies its responsibilities under NEPA and related laws and authorities, and allows the Parish to use Program funds.
OBJECTIONS TO RELEASE OF FUNDS
The Office of Community Development (OCD) will consider objections to its release of funds and the Parish’s certification received by October 30, 2012 or a period of fifteen days from its receipt of the request (whichever is later) only if they are on one of the following bases: (a) the certification was not executed by the Certifying Officer or other officer of the Parish approved by the Office of Community Development (OCD); (b) the Parish has omitted a step or failed to make a decision or finding required by HUD regulations at 24 CFR Part 58; (c) the grant recipient or other participants in the project have committed funds or incurred costs not authorized by 24 CFR Part 58 before approval of a release of funds by the OCD; or (d) another Federal agency acting pursuant to 40 CFR Part 1504 has submitted a written finding that the project is unsatisfactory from the standpoint of environmental quality. Objections must be prepared and submitted in accordance with the required procedures (24 CFR part 58, sec. 58.76) and shall be addressed to Office of Community Development, 150 North Third Street, Suite 700, Baton Rouge, Louisiana 70801. Potential objectors should contact OCD to verify the actual last day of the objection period.

Audit Report: Rowland Perry with the firm of Johnston, Perry, Johnson & Associates Certified Public Accountant, presented the audit report for the year ending December 31, 2011. He stated that the audit was a standard unmodified audit report. The audit shows the Convention and Visitors Bureau with a designated fund balance of $346,899.00 and an unreserved fund balance of $4,633,197.00.
It was moved by Sue Nicholson and seconded by Don O’Toole to accept the 2011 audit report as presented by Rowland Perry of Johnston, Perry, and Johnston & Associates. Motion carried by a vote of eight (8) affirmative votes to zero (0) negative votes.
Other Business: Kyle Edmiston, Assistant Secretary, Louisiana Office of Tourism spoke to the board regarding the effect of the 2012 Louisiana Legislative Session on the 2012-2013 tourism budget. He requested that the board contact their local Legislators to ask them for the 2013-2014 session to let the $7,000,000.00 in pass through funding the Super Bowl Game and the Women’s Final Four Games expire.
Minutes: It was moved by Lane Howell and seconded by Don O’Toole to approve the June 18, 2012 minutes as read. Motion carried by a vote of eight (8) affirmative votes to zero (0) negative votes.

Alana Cooper reported that the budgeted amount for the Special Promotion line item is $800,000.00 which $371,349.00 is already dedicated leaving a balance for July 10, 2012 grant period of $428,651.00.
Don O’Toole reported that the Tourism Development Grant Committee reviewed the thirteen (13) applications for funding with a total request of $831,792.00. He stated that the Tourism Development Grant Committee approved the following request in the total amount of $337,700.00.

**The Chennault Aviation Military Museum and the Louisiana Purchase Zoological Society requested funds for the rental of billboards. The committee recommended that Alana Cooper negotiate a contract with Lamar Advertising for four (4) new billboards along I-20 to advertising the Chennault Aviation Military Museum and the Louisiana Purchase Gardens and Zoo.
**Rebel BXM promoter requested a grant in the amount of $166,200.00 for the construction of a BMX race track in West Monroe. The committee expressed interest in looking into the project, but needed more information on the cost of construction and a certification of a 501(c) organization.
**Twin Cities’ Krewe of Janus requested funds for marching bands, prize money and out of state advertising for the 2013 Mardi Gras Parade. The board dedicated $6,000.00 for bands and $5,000.00 in prize money. The board dedicated $20,000.00 for out of state advertising to be placed by the Convention and Visitors Bureau.
**Blankenship Baseball requested funds to host a Northeast Shootout Baseball Tournament in West Monroe. The committee recommended giving them a sponsorship out of the Convention Service budget in the amount of $2,000.00.
It was moved by Don O’Toole and seconded by Jerry Edmondson to approve the Tourism Development Grant Committee’s recommendation as presented. Motion carried by a vote of eight (8) affirmative votes to zero (0) negative votes.
Don O’Toole reported that the committee discussed a grant to the Downtown Art Fest which has not requested the final 25% payment in the amount of $625.00. The year grace period has expired for them to request the funds and the committee recommends that the funds go back into the special Promotion budget.
Don O’Toole reported that funds of $3,843.00 were in excess of what was needed for the 2010 through 2012 National Archery Shooters Tournament. He recommends that the funds go back into the Special Promotion budget.
It was moved by Don O’Toole and seconded by Jerry Edmondson to release the dedicated funds of $625.00 to the Downtown Art Fest and funds of $3,843.00 dedicated for the 2010-2012 National Archery Shooters tournament to go back into the Special Promotion budget. Motion carried by a vote of eight (8) affirmative votes to zero (0) negative votes.
The committee reviewed a payment request from the Twin Cities’ Krewe of Janus Mardi Gras Parade for funds of $6,000.00 dedicated to pay for bands to perform in the Mardi Gras Parade.
It was moved by Jerry Edmondson and seconded by Ryan Roark to release the funds of $6,000.00 to the Twin Cities’ Krewe of Janus to pay for bands that performed in the 2012 Mardi Gras Parade. Motion carried by a vote of eight (8) affirmative votes to zero (0) negative votes.

Committee Reports:
Bid Committee Meeting – Gretchen Kovac reported that the bid committee met on August 20, 2012 at 2:00 p.m. to listen to four (4) advertising agencies presentation to advertise the 2012 Christmas on the River. She reported that the following agencies presented proposals to the bid committee to advertise Christmas on the River within a $42,500.00 advertising budget at their discretion.

She reported that the committee recommends that Active Media represented by Bradley Wilkinson be awarded the 2012 Christmas on the River advertising.
It was moved by Gretchen Kovac and seconded by Don O’Toole to approve Active Media, Bradley Wilkinson to advertise the 2012 Christmas on the River in the amount of $42,500.00. Motion carried by a vote of seven (7) affirmative votes to zero (0) negative votes.
Tourism Development Committee – Sammy Gordy reported the Tourism Development Grant Committee met at 3:45 to discuss a final payment request in the amount of $1,250.00 from the City of Monroe Deltafest.
It was moved by Sammy Gordy and seconded by Don O’Toole to approve the final payment request in the amount of $1,250.00 to the City of Monroe Deltafest. Motion carried by a vote of seven (7) affirmative votes to zero (0) negative votes.
Minutes: It was moved by Lane Howell and seconded by Gretchen Kovac to approve the July 16, 2012 minutes as read. Motion carried by a vote of seven (7) affirmative votes to zero (0) negative votes.
Treasurer’s Report: The board reviewed the itemized deposits and disbursement showing the following balances on hand as of July 31, 2012.

General Fund $4,820,380.05

Travel Plans: It was moved by Lane Howell and seconded by Gretchen Kovac to approval the additional travel plans for September 2012 through January 2013. Motion carried by a vote of seven (7) affirmative votes to zero (0) negative votes.
Executive Director Report: Alana Cooper requested board approval to dedicate $100,000.00 for purchase of lights, installation, take down, and other needed expenses for the 2012 Christmas on the River.
It was moved by Sue Nicholson and seconded by Lane Howell to dedicate $100,000.00 in the Special Promotion budget for purchase of lights, installation, take down, and other needed expenses for the 2012 Christmas on the River.
