The equipment shall consist of a combination sewer cleaner mounted on a vehicle chassis capable of removing stones, grit, grease, sludge and other debris from sanitary sewer and/or storm drain lines.

It was moved by Janet Rutledge and seconded by Jerry Edmondson to approve the January 2012 Tourism Development Grant requests for funding as presented. Motion carried by a vote of eight (8) affirmative votes to zero (0) negative votes.
Janet Rutledge reported that the Tourism Development Grant Committee reviewed the following grants for final payment.
1. Chennault Aviation & Military Museum requested final payment of $5,000.00 granted in July 2011 for an electronic sign. Alana Cooper reported that the Museum have a dedicated balance of $6,197.00 for a grant awarded in January 2011 for an electronic sign that is still in dedicated funds that has not been requested.
It was moved by Sammy Gordy and seconded by Janet Rutledge to approve the $5,000.00 request for final grant payment dedicated for the purchase of an electronic sign for the Museum. The board requested that the director get in touch with the Museum to clarify why the dedicated grant balance of $6,197.00 from January 2011 was not requested. Motion carried by a vote of eight (8) affirmative votes to zero (0) negative votes.
2. The Biedenharn Museum and Garden requested dedicated funds in the amount of $1,500.00 for the printing of the Biedenharn’s Christmas Church flyer.
It was moved by Janet Rutledge and seconded by Sammy Gordy to approve payment in the amount of $1,500.00 to the Biedenharn Museum and Garden for the printing of a Christmas Church flyer for mail-outs. Motion carried by a vote of eight (8) affirmative votes to zero (0) negative votes.
3. USTPA 2011 Wrangler World Championship requested final payment in the amount of $5,000.00 for funds dedicated for Team Penning Show.
It was moved by Janet Rutledge and seconded by Sammy Gordy to approve the final grant payment in the amount of $5,000.00 to the USTPA. Motion carried by a vote of eight (8) affirmative votes to zero (0) negative votes.
Other Business: Alana Cooper report that the Convention & Visitors Bureau will host the FLW Ever Start Fishing Tournament in November 2012. She requested board approval to dedicate $15,000.00 to cover the expenses for bringing in and hosting the tournament.

Executive Director’s Report: Alana Cooper reported that the Convention & Visitors Bureau has finished payment of dedicated funds to the N/E Louisiana Delta African American Heritage Museum for the construction. She stated that the account still has a balance of $13,777.97 due to the money was in an interest bearing account. She reported that she spoke with Grant Schlueter, Bond Attorney, to see what the balance of the funds could be used for. She stated that he told her that the funds can used for the purchase of any fixed assets that has been purchased or needed to be purchase.
Alana Cooper reported that Elmer Noah, Board Attorney, stated that the Convention & Visitors Bureau should do Co-operative Endeavor Agreements with any governmental entities that we grant a large amount of funds.
She stated that a Co-operative Endeavor Agreement is in the process of being drawn up between ULM/University Park and the Convention & Visitors Bureau in the amount of $100,000.00. She stated that ULM/University Park requested that the end date be extended by one (1) year.
It was moved by Gretchen Kovac and seconded by Sue Nicholson to extend the date on the Cooperative Endeavor Agreement between ULM/University Park and the Convention & Visitors Bureau by one (1) year. Motion carried by a vote of eight (8) affirmative votes to zero (0) negative votes.

Committee Reports:
Tourism Development Grant Committee – Janet Rutledge reported that the Tourism Development Grant Committee met on February 27, 2012 at 3:45 p.m. to review a final payment request in the amount of $6,250.00 from the Northeast Louisiana Children’s Museum. She stated that after reviewing the request the committee made the following recommendation.
It was moved by Janet Rutledge and seconded by Sammy Gordy to approve the Northeast Louisiana Children’s Museum final payment request in the amount of $6,250.00. Motion carried by a vote of eight (8) affirmative votes to zero (0) negative votes.
Other Business: Alana Cooper reported that the Board dedicated funds in the amount of $12,000.00 per year for three (3) years to host the National Archery Shooters Tournament. She stated that the Convention & Visitors Bureau hosted an extra tournament in 2011 which cost $8,000.00. She requested board approval to dedicate $8,000.00 back to the special promotion budget to cover expenses for the 2012 tournament that was held February 24-26.
It was moved by Sue Nicholson and seconded by Gretchen Kovac to dedicate $8,000.00 to the Special Promotion budget to cover expenses for the 2012 National Archery Shooter Tournament held February 24-26, 2012 in Monroe. Motion carried by a vote of eight (8) affirmative votes to zero (0) negative votes.
Alana Cooper reported that advertising for proposals was sent out to local advertising agencies to advertise the 2012 DeltaFest in the out of state market of Mississippi and Arkansan regions in the amount of $20,000.00. She stated that the following proposals were received.

She reported that after reviewing the two (2) proposals she recommend that the board approves the proposal of Newcomer, Morris and Young, Inc., in the amount of $19,990.00.
It was moved by Gretchen Kovac and seconded by Sammy Gordy to approve the proposal of Newcomer, Morris and Young, Inc., in the amount of $19,990.00 to do the advertising for the 2012 DeltFest to be held April 26-29, 2012 at the Monroe Civic Center.

Motion carried by a vote of seven (7) affirmative votes to zero (0) negative votes.
Janet Rutledge also reported that the committee reviewed a final payment request in the amount of $4,000.00 from the Louisiana Cattlemen’s Association. The committee recommended that upon receipt of completed paperwork from the Louisiana Cattlemen’s Association that the final payment in the amount of $4,000.00 be paid. It was moved by Janet Rutledge and seconded by Sue Nicholson that upon receipt of completed paperwork from the Louisiana Cattlemen’s Association that the final payment request in the amount of $4,000.00 be paid. Motion carried by a vote of seven (7) affirmative votes to zero (0) negative votes.
Other Business: Alana Cooper reported that it was time to bid on the Archery Shooters Association National Tournament. Alana stated that she had met with Sheriff Elect Jay Russell and Captain David Jones of the Ouachita Parish Sheriff’s Department. There were 1100 shooters in the March tournament. The Sheriff’s Department wants to maintain their agreement to host this tournament. The years to be negotiated are 2013, 2014, and 2015. Alana requested the Board dedicate $12,000 per year for three (3) years to host the Archery Shooters Association National Tournament.

Proceedings of the Board of Commissioners of the West Ouachita Sewerage District No. 5, Ouachita Parish, Louisiana. Taken at the office of the district, 327 Wallace Road, West Monroe, LA. Monday, July 9, 2012 (regular meeting).
Members present: Fred Hall, Don Leach, Ralph Owens
Members absent: None
Others present: Francis Huffman-Luffey, Huffman, Ragsdale & Soignier, APAC.; Jerry Lazenby, Lazenby & Associates, Inc.; Terry Cox, district manager; Scotty Robinson, Ouachita Parish Police Jury.
Commissioner Hall called the meeting to order promptly at 11:00 a.m.
The first item on the agenda was the engagement letter for years ended August 31, 2012, 2013 and 2014, with Luffey, Huffman, Ragsdale & Soignier, APAC. Commissioner Leach made a motion to accept them as our auditor for the said years and Commissioner Owens seconded the motion. The motion passed by a unanimous voice vote.
Next, manager Terry Cox reported that the sanitary sewer collection system recently constructed to serve the Westfields Duplex Subdivision has been inspected and found to be satisfactory and that all required documentation had been received from the developer and his engineer. Motion by Commissioner Leach to accept the wastewater collection system for operation and maintenance with a one year guarantee of materials and workmanship from the developer. Motion seconded by Commissioner Owens. Motion unanimously approved by a voice vote following a brief discussion.
Engineer Lazenby and Manager Terry Cox reported that the plans and specifications for the Wallace Dean duplexes have been reviewed and it was recommended that the project be granted preliminary approval subject to comments in letter dated June 20, 2012. Motion by Commissioner Leach to grant preliminary approval of the plans and specifications for the Wallace Dean duplexes by Johnson & Autonberry, LLC, with the stipulation that the WOSD No. 5 would not be liable for any sewer backup into the duplex housing units since the finished floor elevation of several of the units is below the top of existing manholes along Wallace Dean Road. Motion seconded by Commissioner Owens. Motion unanimously approved by voice vote following a short discussion.
Engineer Lazenby and Manager Terry Cox reported that the plans and specifications for the Flanagan Road duplexes had been reviewed and it is recommended that the project be granted preliminary approval. Motion by Commissioner Owens to grant preliminary approval of the plans and specification for the sewer collection system improvements for the Flanagan Road duplexes by MFD Properties LLC. Motion seconded by Commissioner Leach. Motion unanimously approved by voice vote following a short discussion.
Manager Terry Cox and Engineer Jerry Lazenby, PE, discussed the plans for installing a sanitary sewer collection line along Alice Street (a short street between Lilac Street and Washington Street) to serve several single family residential units that currently have inadequate sewer service. Motion by Commissioner Leach to authorize the district's manager and engineer to obtain bids from interested contractors and to pursue the construction of the project, projected to cost approximately $50,000. Motion seconded by Commissioner Owens. Following a short discussion, the motion was passed by a unanimous voice vote.
Next, the discussion of the proposed agreement by and between WOSD No. 5 and the city of West Monroe for sewer treatment. In order to have a clear understanding of the terms and conditions, Commissioner Leach made a motion that the board meet with representatives of West Monroe and discuss the proposed agreement. Motion seconded by Commissioner Owens. Motion unanimously approved by a voice vote.
Manager Terry Cox reported to the board that Mr. Donald Plummer was requesting permission to construct a storage garage within the existing sewer servitude at 104 Jonathan Street. Mr. Plummer is agreeable to terms and conditions established by the district. Motion by Commissioner Leach to allow Mr. Plummer to encroach upon the district's sewer servitude at this location as a special exemption subject to Mr. Plummer's executing a document expressing the district's terms and conditions. Motion seconded by Commissioner Leach. Motion unanimously approved by a voice vote following a brief discussion.
The final item was cash disbursements. Mr. Cox presented the disbursements for the month and a brief discussion followed. Commissioner Leach made a motion to accept them as reasonable and necessary and Commissioner Owens seconded the motion. The motion passed by a unanimous voice vote.
No further business being presented the meeting adjourned at 12:40 p.m.

A special meeting of the Board of Commissioners, Hospital Service District No. 1 of the Parish of Ouachita, State of Louisiana was called to order at 11:45 A.M. at the Wellness Center, 3215 Cypress Street, West Monroe, Louisiana, by Steve Hall, Chairman of the Board of Commissioners. All references to the “District” refer to the Hospital Service District No. 1 of the Parish of Ouachita, State of Louisiana. All references to the “Foundation” refer to the Living Well Foundation.

APPROVAL OF MINUTES OF DISTRICT BOARD MEETING OF JUNE 12, 2012
After review of the minutes of the District’s June 12, 2012 Board meeting, a motion was made by Mr. Spencer and seconded by Mr. Mulhearn to adopt the minutes as presented and the motion was unanimously approved.
AMENDMENT OF AGENDA
Mr. Mulhearn moved to take up the following matter not on the agenda: "Executive Session For the Purposes of Strategic Planning, including the Discussion of the Appointment of a New Commissioner" and stated that the purpose for the addition to the agenda is to address a matter not on the agenda that it is in the best interest of the District to address immediately. Mr. Spencer seconded the motion.
The Chairman declared an opportunity for public comment on the motion. There were no public comments. The Board proceeded to a vote, and the motion passed with the unanimous approval of the Commissioners present, whereupon the Board took up the matter.
EXECUTIVE SESSION FOR PURPOSES OF STRATEGIC PLANNING, INCLUDING THE APPOINTMENT OF A NEW COMMISSIONER
A motion was made by Mr. Mulhearn, seconded by Mr. Spencer, and unanimously approved for the Board of Commissioners to go into executive session for the purposes of strategic planning and discussion of the appointment of a new Commissioner.
Mr. Mulhearn motioned, Mr. Spencer seconded, and the Commissioners present approved the motion to go back into public session.
OPENING OF BIDS FOR THE EXPANSION OF THE WELLNESS CENTER
The following representatives were in attendance to present bid packets for the future expansion of the Wellness Center:

APPROVAL/AWARDING OF BID FOR THE EXPANSION OF THE WELLNESS CENTER
After due discussion, Mr. Hall proposed, Mr. Mulhearn seconded, and the Board of Commissioners unanimously adopted the following resolution:
NOW COME the Members of the Hospital Service District No. 1 of Ouachita Parish, and collectively resolved and attested:
A duly scheduled and noticed meeting of the Hospital Service District No. 1 of Ouachita Parish was held on June 26, 2012, with a quorum being present, namely Steve Hall, Mike Mulhearn, Randy Gilley, and Patrick Spencer;
After being duly advertised and noticed, six bids were received in connection with the expansion at the Wellness Center;
The bids received were from Breck Construction Company Inc., Don M. Barron Contractor Inc., Don Leach Construction, Lincoln Builders of Ruston, Traxler Construction, and Mann Construction Inc;
The Members present for the meeting voted unanimously to accept the low bid from Breck Construction and to undertake the contract and project with Breck Construction, to include a 10 month or 300 calendar day time of completion from the start date, along with a $750 per day liquidation of damages, and authorized the Chairman to sign a standard form AIA construction contract and related documents with Breck Construction, consistent with such bid and such terms and containing such other terms and conditions as the Chairman may approve in his discretion after consultation with counsel, such approval to be conclusively evidenced by his signature thereon;
THUS APPROVED AND RESOLVED by the undersigned Members of the Hospital Service District No. 1 of Ouachita Parish.
