COMMISSIONER-IN-CHARGE
INSTRUCTION CLASS
The School of Instruction for those Certified Commissioners who wish to serve as Commissioner-In Charge in Ouachita Parish for the year 2008 will be held:
Monday, November 19, 2007, 12:00 Noon, at the Monroe Civic Center (FT Miro Room)
All interested persons are invited to attend. In Accordance with LA R.S. 18:424B, you must have the following qualifications to serve as a Commissioner-In-Charge:
1. He shall be a qualified voter in the parish in which he is to serve who is not entitles to assistance in voting.
2. He shall not be a candidate for election to public office nor be a member of the immediate family of a candidate for election to public office in the precinct in which he serves.
3. He shall not have been convicted of an election offense.
4. He shall have successfully completed a general course of instruction for Commissioners-In-Charge and provided his correct party affiliation to the Clerk.
5. He shall have served as a Commissioner in at least two (2) elections during the last four years.

NOTICE
Notice is hereby given, pursuant to Article IV, Section 21(D)(1) of the Louisiana Constitution, that on October 29, 2007, Entergy Louisiana, LLC. (“ELL”), an electric public utility having facilities to provide retail electric service to residential, commercial, industrial, and governmental customers in forty-six Parishes of the State of Louisiana, filed with the Louisiana Public Service Commission (“LPSC”), pursuant to Article IV, Section 21(D)(3) of the Louisiana Constitution and Title 45, Section 1163.1 of the Louisiana Revised Statutes, a modified Formula Rate Plan (“FRP”) rider schedule that implemented, effective the first billing cycle of November 2007 (October 29), a $7.1 million annual FRP revenue decrease pursuant to the LPSC General Order dated October 12, 2007 and Order U-28919, and in conjunction with the FRP proceedings currently before the LPSC in Docket No. U-20925 (TY 2006). This FRP rate reduction of approximately $7.1 million reflects: 1) a $5.2 million reduction for ELL’s compliance with the LPSC’s General Order dated October 12, 2007, which requires that 50% of franchise fees derived from electric utilities shall be placed in base rates and collected from all of the electric utilities’ ratepayers and the other 50% of franchise fees shall be borne only by the ratepayers within the municipality charging said fees and shall be noted on those ratepayers’ bills as a line item; and 2) a $1.8 million incremental rate reduction associated with Entergy Louisiana, Inc.’s conversion to a Limited Liability Corporation pursuant to LPSC Order U-28919.
This decrease of $7.1 million will have the following effects upon ELL bills for customers not subject to the line item franchise fee based upon November 2007 Fuel Adjustment Charges. A bill for Residential Service for 1,000 kWh would decrease $0.37 from $87.59 to $87.22. A commercial customer’s bill for Small General Service for 12,500 kWh would decrease $5.49 from $1,233.82 to $1,228.33. An industrial customer’s bill for Large General Service for 1,000 kW and 500,000 kWh would decrease $124.74 from $35,633.15 to $35,508.41.
In municipalities that charge a two percent franchise fee, the change would have the following effects on customers’ bills. A bill for Residential Service for 1,000 kWh would increase $0.50 from $87.59 to $88.09. A bill for Small General Service for 12,500 kWh would increase $6.79 from $1,233.82 to $1,240.61. A bill for Large General Service for 1,000 kW and 500,000 kWh would decrease $124.74 from $35,633.15 to $35,508.41. The slips containing the names of persons listed were then places in a separate envelope, which was then sealed and the words JURY NUMBER ONE written thereon and placed in said box labeled “JURY BOX.” The Jury Box and the General Venire Box were then locked and sealed and delivered to the custody of the Clerk of said Court, subject to the orders of Court.

Notice to Intention of Hospital Service District No. 1 of the Parish of Ouachita, State of Louisiana to select a fiscal agency.
Notice is hereby given that Hospital Service District No. 1 of the Parish of Ouachita, State of Louisiana (the “District”) intends to select a fiscal agency under the Local Depositories Law, La. R.S. 39:1211 et seq. A copy of the District’s Request for Proposal for Fiscal Agent may be obtained from Pat Thompson at (318) 329-8823 or pthompson@wellnesscenterwm.com
Each and every bank that is domiciled in or has a branch office in the Parish of Ouachita is invited to submit a proposal. Proposals must be submitted in writing, sealed, and received by Thursday, November 29, 2007 at 6:00 p.m. at Hospital Service district No. 1, 3215 Cypress St., West Monroe, La. 71291, whether delivered by hand or by mail. The district reserves the right to reject any and all proposals.
The fiscal agency contract shall be for the period from January1, 2008 to December 31, 2008, with the district having options for two (2) additional one (1) year periods. The terms and conditions of the fiscal agency contract shall be subject to the Local Depositories Law and shall provide for deposits and checks and contain such other terms and conditions as are in the proposal that the District funds best meets its needs.

All bids must be accompanied by bid security equal to five (5%) of the base bid and all additive alternates, and must be in the form of a certified check, cashier’s check or bid bond written by a company licensed to do business in Louisiana, countersigned by a person who is residing in this state. No Bid Bond indicating an obligation of less than five percent (5%) by any method in acceptable.
The successful Bidder will be required to furnish a performance and payment bond written by a company licensed to do business in Louisiana, and shall be countersigned by a person who is contracted with the surety company or bond issuer as agent of the company or issuer, and who is licensed as an insurance agent in this State, in an amount equal to the 100% of the contract amount.
No bid may be withdrawn for a period of thirty (30) days after receipt of bids, except under the provisions of Act 111 of 1983.
A mandatory pre-bid conference will be Monday, November 26, 2007, at 9:00 A.M., at the job site on the Riser Middle School campus. Bids shall be accepted only from Contractors who attend the Pre-bid Conference.
The Owner reserves the right to reject any and all bids.
