CALL TO ORDER
A special meeting of the Board of Commissioners, Hospital Service District No. 1 of the Parish of Ouachita, State of Louisiana was called to order at 4:00 P.M. in The Wellness Center, 3215 Cypress Street, West Monroe, Louisiana, by Violet Liner, Chairman of the Board of Commissioners.

Mr. Mulhearn gave the invocation.

OPPORTUNITY FOR PUBLIC COMMENT
There were no comments from the public.
REMARKS FROM THE CHAIRMAN OF THE DISTRICT
Glenwood Resolution Authority will meet on Thursday, June 21, and Steve Hall will attend in Ms. Liner’s absence.
Ms. Liner reported that the Legislative Audit report for year ending December 31, 2006 should be complete by June 30, 2007.
Plans are also underway for The Wellness Center to generate their first payroll independent from IASIS Glenwood Regional Medical Center on August 15, 2007.
EXECUTIVE SESSION FOR THE PURPOSES OF STRATEGIC PLANNING, INCLUDING DISCUSSION OF THE FUTURE OPERATION AND POSSIBLE RENOVATION OF THE WELLNESS CENTER
A motion was made by Mr. Spencer, seconded by Mr. Nelson, and unanimously approved for the Board of Commissioners to go into executive session for the purpose of strategic planning, including discussion of the future operation and possible renovation of The Wellness Center.
All present were allowed to remain.
A motion was made by Mr. Hall seconded by Mr. Spencer and unanimously approved for the Board of Commissioners to go back into regular session.
TRANSACTION OF ANY OTHER BUSINESS THAT MAY PROPERLY COME BEFORE THE MEETING
It was decided that the June 29th regularly scheduled monthly meeting of the Board of Commissioners should be rescheduled to Monday, July 2, 2007 at 4:30 p.m. at The Wellness Center, 3215 Cypress Street, West Monroe, Louisiana.
ADJOURNMENT
With no further business to discuss, Ms. Liner declared the meeting adjourned.

NOTICE OF SPECIAL ELECTION
Pursuant to the provisions of a resolution adopted by the Police Jury of the Parish of Ouachita, State of Louisiana (the “Governing Authority”), acting as the governing authority of the Parish of Ouachita, State of Louisiana (the “Parish”) on April 16, 2007, NOTICE IS HEREBY GIVEN that a special election will be held within the Parish on SATURDAY, JULY 21, 2007, and at the said election there will be submitted to all registered voters qualified and entitled to vote at the said election under the Constitution and laws of this State and the Constitution of the United States, the following proposition, to-wit:
PARISHWIDE PROPOSITION
(OUACHITA CORRECTIONAL CENTER MILLAGE)
SUMMARY: 5 YEAR 8.6 MILLS PROPERTY TAX FOR THE PURPOSE OF IMPROVING, MAINTAINING, OPERATING AND EQUIPPING THE OUACHITA CORRECTIONAL CENTER, AND AUTHORIZATION OF 5 YEAR 0.6 MILLS PROPERTY TAX FOR THE PURPOSE OF MAKING MAJOR IMPROVEMENTS, REPAIRS AND/OR RENOVATIONS TO THE EXISTING OUACHITA PARISH CORRECTIONAL CENTER BUILDINGS AND STRUCTURES TOGETHER WITH THE NECESSARY EQUIPMENT, FURNISHINGS AND FINANCING THEREFOR.
Shall the Ouachita Parish Police Jury, as governing authority of the Parish of Ouachita, State of Louisiana (the “Parish”), be authorized to levy a special tax of eight and six tenths (8.6) mills on all property subject to taxation in said Parish for a period of five (5) years, beginning with the year 2007, and ending with the year 2011, for the purpose of improving, maintaining, operating and equipping the Ouachita Correctional Center, and, shall said Parish be further authorized to levy a special tax of six tenths mils (0.6 mils) on all property subject to taxation in said Parish for a period of five (5) years, beginning with the year 2007, and ending with the year 2011, for the purpose of making major improvements, repairs and/or renovations to the existing Ouachita Correctional Center buildings and structures together with the necessary equipment, furnishings and financing therefore? It is estimated (as of the date of this proposition) that the amount reasonably expected to be collected from the levy of the 8.6 mils tax above for the first year will be approximately $6,168,858, and the amount reasonably expected to be collected from the levy of the 0.6 mils tax above for the first year will be $430,385.
The said special election will be held at the following polling places situated within the Parish, which polls will open at six o’clock (6:00) a.m., and close at eight o’clock (8:00) p.m., in accordance with the provisions of La. R.S. 18:541, to-wit:

The said special election will be held in accordance with the applicable provisions of Chapter 5 and Chapter 6A of Title 18 of the Louisiana Revised Statutes of 1950, as amended, and other constitutional and statutory authority, and the officers appointed to hold the said election, as provided in this Notice of Special Election, or such substitutes therefore as may be selected and designated in accordance with La. PUBLIC NOTICE
We are required to monitor your drinking water for disinfection byproducts (total trihalomethanes [TTHMs] and haloacetic acids-five [HAA5]) every calendar quarter.
Results of regular monitoring are an indicator of whether or not your drinking water meets health standards. During the monitoring period of October 1, 2006 through December 31, 2006, the Pine Bayou Water System did not monitor for TTHMs and HAA5, and therefore we cannot be sure of the quality of your drinking water during that time. Please share thisinformation with all the other people who drink this water, especially those who may not have received this notice directly (for example, people in apartments, nursing homes, schools, and businesses). You can do this by posting this notice in a public place or distributing copies by hand or mail.
This violation occurred because of the failure to collect the required samples for total trihalomethanes (TTHMs) and haloacetic acids-five (HAA5) from the water system’s distribution system. Necessary action has been taken to prevent this violation from recurring. If you have any questions, please contact the office at 318-397-2835.

The project consists of a new 6,635 sq. ft. Juvenile Detention Alternative Center (Administrative Building) and 2,700 sq. ft. Female Housing addition to the existing housing buildings. The project site is located at 4820 South Grand Street, Monroe, Louisiana.
A Mandatory pre-bid conference will be held at 9:00 a.m. on Friday, July 27, 2007 at the project site at 4820 South Grand Street for all interested Contractors. Bids shall only be accepted from General Contractors who attend this meeting.
All bids shall be lump-sum and must be accompanied by bid security equal to five percent (5%) of the sum of the base bid and all alternates, and must be in the form of a certified check, cashier's check or Bid Bond form written by a surety company licensed to do business in Louisiana, signed by the surety's agency or attorney?in?fact, and countersigned by a person who is under Contract with surety as a licensed agency in this State and who is residing in this State. Surety must be listed on the current U.S. Department of the Treasury Financial Management Service list of approved bonding companies as approved for an amount equal to or greater than the amount for which it obligates itself in the Bond, or must be a Louisiana domiciled insurance company with at least an A? rating in the latest printing of the A.M. Best's Key Rating Guide. If surety qualifies by virtue of its Best's listing, the amount of the Bond may not exceed ten percent of policyholders' surplus as shown in the latest A.M. Best's Key Rating Guide. The Bid Bond shall be in favor of the Ouachita Parish Police Jury, and shall be accompanied by appropriate power of attorney. No Bid Bond indicating an obligation of less than five percent (5%) by any method is acceptable.
The successful Bidder shall be required to furnish a Performance and Payment Bond written by a company licensed to do business in Louisiana, in an amount equal to 100% of the Contract amount. Surety must be listed currently on the U.S. Department of Treasury Financial Management Service List (Treasury List) as approved for an amount equal to or greater than the contract amount, or must be an insurance company domiciled in Louisiana or owned by Louisiana residents. If surety is qualified other than by listing on the Treasury list, the contract amount may not exceed fifteen percent of policyholders' surplus as shown by surety's most recent financial statements filed with the Louisiana Department of Insurance and may not exceed the amount of $500,000. However, a Louisiana domiciled insurance company with at least an A? rating in the latest printing of the A.M. Best's Key Rating Guide shall not be subject to the $500,000 limitation, provided that the contract amount does not exceed ten percent of policyholders' surplus as shown in the latest A.M. Only Contractors licensed according to Contractor's Licensing Law, R.S. 37:2151-2163, shall be considered if his bid is greater than $50,000.00 and shall show his license number on exterior of Bid Envelope and above his signature or the signature of his duly authorized representative.
No bid may be withdrawn for a period of forty-five (45) days after receipt of bids per Louisiana Revised Statue 38:2215(A).

We are required to monitor your drinking water for Lead and Copper.?Results of regular monitoring are an indicator of whether or not your drinking water meets health standards. During the monitoring period of August 1, 2006 through November 30, 2006, the Three S MHP Water System did not monitor for Lead and Copper, and therefore we cannot be sure of the quality of your drinking water during that time. Please share this information will all the people who drink this water. This violation occurred because of the failure to collect the required samples for Lead and Copper from the water system’s distribution system. Necessary action has been taken to prevent this violation from recurring. In accordance with the laws of the State of Louisiana, and with particular reference to the provisions of Title 30 of Louisiana Revised Statutes of 1950, a public hearing will be held in the Hearing Room, 1st Floor, LaSalle Building, 617 North 3rd Street, Baton Rouge, Louisiana, at 9:00 a.m. on TUESDAY, AUGUST 14, 2007, upon the application of RIVERSTONE ENERGY OPERATING, LLC.

COTTON VALLEY FORMATION, RESERVOIR A
1. To create one (1) additional drilling and production unit for the exploration for and production of gas and condensate from the Cotton Valley Formation, Reservoir A.
2. To force pool and integrate all separately owned tracts, mineral leases and other property interests within the proposed unit with each tract sharing in unit production on a surface acreage basis of participation.
3. To designate an operator of, and unit well for, the proposed unit as may be appropriate.
4. To redefine the Cotton Valley Formation, Reservoir A, INSOFAR AND ONLY INSOFAR as to the additional unit proposed herein, as being that gas and condensate bearing interval encountered between the depths of 8,950' and 9,500' (electrical log measurements) in the Chevron USA Inc.-Effie T. Barnes et al No. 1 Well, located in Section 4, Township 18 North, Range 1 East.
5. To extend, except as in conflict herewith, the provisions of Office of Conservation Order No. 746-J, effective September 14, 2004, an all applicable Statewide Orders, to the proposed additional unit.
6. To consider such other matters as may be pertinent.
