PROCLAMATION
WHEREAS, PHILLIP ODOM has submitted his Notice of Resignation
representing District G of the Ouachita Parish School Board; and,
WHEREAS, his resignation has caused a vacancy in office and an election
must be ordered and called to fill the unexpired term of office exceeding
one year; and,
WHEREAS, the Ouachita Parish School Board is given authority and
responsibility under the provisions of La. R.S. 18:602 to appoint a person
to fill the vacancy who meets the qualifications of the office, and to call a
special election to fill the vacancy.
NOW,THEREFORE, the members of the Ouachita Parish School Board
at a regular meeting duly held and properly noticed on the 25th day of
May, 2007, resolved that LOUIS J. ST. AMANT be appointed to fill the
District G School Board seat until a special election can be held to fill the
vacancy.
BE IT FURTHER RESOLVED that an election be called and held with
the Primary election date set for October 20, 2007, and the General election
date set for November 17, 2007, with qualifying dates set for September
4, 5 and 6, 2007.
By motion of Board Member Carey Walker, duly seconded by Board
member Jerry R. Hicks, the above proclamation was adopted by the following
vote on the 25th day of May, 2007.

CERTIFICATE
I herby certify that the foregoing Proclamation was adopted by the
Ouachita Parish School Board in regular session, duly convened on the

of Ward Five Healthcare Foundation.

RATIFICATION OF ENGAGEMENT OF CPA FIRM TO PREPARE
HSD FINANCIALS FOR LEGISLATIVE AUDITOR
A motion was made by Mr.Mulhearn to adopt the following resolution:
RESOLVED, that the engagement letter dated March29, 2007 between
the District and Luffey Huffman & Monroe, A Professional Accounting
Corporation relating to the compilation of the financial statements of the
District for the year ending December 31, 2006 is ratified, confirmed, and
approved.
Mr. Nelson seconded the motion to adopt the foregoing resolution,
which carried unanimously.
RATIFICATION OF ANCILLARY SERVICES AGREEMENT WITH
IASIS GRMC RELATING TO THE WELLNESS CENTER
Mr.Mulhearn moved,Mr.Hall seconded, and the Commissioners unanimously
approved the following resolution:
WHEREAS, the Board of Commissioners previously authorized the
Chairman to execute an ancillary services agreement with IASIS Glenwood
Regional Medical Center, L.P. (“IASIS”) relating to The Wellness
Center and the Chairman has done so;
RESOLVED, that the Ancillary Services Agreement effective as of January
31, 2007 between the District and IASIS is hereby ratified, confirmed
and approved.
APPROVAL OF LOGO FOR THE WELLNESS CENTER
Because of discontent with the current selection of logos, a decision was
made to defer the approval of logos until the next meeting.
DESIGNATION OF CERTAIN MEMBERS OF THE IASIS GRMC
HOSPITAL BOARD
After due discussion, the Board of Commissioners unanimously approved
the following resolution:
WHEREAS, Schedule 7.1(c) of the Asset Purchase Agreement among
the District, IASIS, and others provides that at all times during IASIS’
ownership of Glenwood Regional Medical Center, the
District shall have the right to designate fifty percent (50%) of the hospital’s
Board of Trustees, which shall be composed of IASIS’s chief executive
officer, members of the hospital’s medical staff, and non-physician
community leaders, and IASIS has decided that the board shall have ten
members;
RESOLVED, that the Board of Commissioners hereby designates the
following five persons to serve as the District-designated members of the
hospital’s Board of Trustees; Ms. Pam Accardo, Mr. Wade Bishop, Mr.
Whitty Hood, Dr. Bob Webber, and Mr. John Mark Wilhite.

PUBLIC NOTICE
Proceedings of the Board of Commissioners of the West Ouachita Sewerage District No. 5, Ouachita Parish, Louisiana. Taken at the office of the district, 327 Wallace Road, West Monroe, Louisiana. Monday, May 14, 2007 (regular meeting).

Commissioner Hall called the meeting to order promptly at 11:00 a.m.
The first item on the agenda was Silmon Estates Subdivision, to consist of 16 single-family lots. Mr. Lazenby presented the plans and specifications for the subdivision and recommended the approval of the plans. Following a brief discussion, Commissioner Leach made a motion to accept the subdivision and Commissioner Owens seconded the motion.
The next item on the agenda was the disposal bids for two (2) pick-up trucks. We received four (4) sealed bids, that were opened at the regular session of the Board of Commissioners. The bids were as follows:

Artie McMullen, 02 Ford, $8,595
Mickey Loftin, 02 Ford, $7,500; 98 Ford, $1,200
Steve Taylor, 02 Ford, $12,550; 98 Ford, $1,525
The trucks were sold to Steve Taylor, as highest bidder. Mr. Cox. will notify Mr. Taylor and take care of the final arrangements of the sale (bids are on file at the office of the district).
The final item was cash disbursements. Mr. Cox presented the disbursements for the month and following a brief review, Commissioner Leach made a motion to accept them as reasonable and necessary and Commissioner Owens seconded the motion.
No further business being presented, the meeting adjourned at 11:45 a.m.

NOTICE
Notice is hereby given, pursuant to article IV, section 21(D)(1) of the Louisiana Constitution, that on May 15, 2007, Entergy Louisiana, LLC (“ELL”) an electric public utility providing retail electric service to customers in forty-three (43) Parishes of the State of Louisiana, filed with the Louisiana Public Service Commission (“LPSC”) its Formula Rate Plan (“FRP”) Rider FRP for the test year ending December 31, 2006 pursuant to the FRP adopted by the Commission in Consolidated Order No. U-29205 RRF 2004.
The filing indicates that the Company requires an annual FRP rider revenue decrease of approximately $6.9 million dollars after reflecting certain adjustments, including several that are due to the extraordinary effects of Hurricane Katrina. The primary adjustments that are reflected in this case include: (1) an increase of approximately $39.8 million annually for unrecovered fixed cost associated with the loss of customers in St. Bernard, Plaquemines, and lower Jefferson parishes; (2) a decrease of approximately $18.7 million annually in the collection of a storm reserve in anticipation of the Commission entering an order in Docket No. U-29203 that approves the current stipulation among ELL, the Commission Staff, and most of the intervenors establishing a securitized storm reserve; (3) a decrease of approximately $4.0 million annually pursuant to FRP Section 3.D to reflect a reduction in additional capacity costs; and (4) a decrease of approximately $24.2 million annually which reflects the termination of interim storm cost recovery in connection with final Commission resolution of Docket No. U-29203. Interim storm recovery will be replaced by permanent storm costs recovery.
Based upon the May billed fuel adjustment, this prospective rate decrease would have the following effects: A typical bill for Residential Service for 1,000 kWh would decrease $0.37 from $102.66 to $102.29. A bill for Small General Service for 50 kW and 12,500 kWh would decrease $5.32 from $1,383.03 to $1,377.71. A bill for Large General Service for 500 kW and 225,000 kWh would decrease $58.49 from $19,329.57 to $19,271.08. Pursuant to FRP section 2.B. these rates or such corrected Rate Adjustments as determined under the FRP may become effective on or after the first billing cycle of September. At this time, ELL expects this annual FRP rate decrease to become effective coincident with the effective date of whatever rate increase is ordered by the Commission in Docket No. U-29203. ELL anticipates that the latter will exceed the former, such that the net effect on customers will likely be a rate increase.
