WEST MONROE HOUSING AUTHORITY
TUESDAY, APRIL 10, 2007
There was a Meeting of the West Monroe Housing Authority held on Tuesday, April 10, 2007, at 7:00 p.m. In the Council Chamber of the West Monroe City Hall.
Those present were Mayor Dave Norris, Aldermen L. O. Yeager, James Bennett, Alice Pearson and Fred Ragland. Absent: James Brian.
There was a motion by Mr. Yeager, seconded by Mrs. Pearson and unanimously carried to approve a resolution to amend the Administrative Plan concerning emergency applications for the elderly/handicapped/disabled. Resolution 559:
A RESOLUTION TO AMEND CHAPTER 4, SECTION 4-13, SUBPARAGRAPH "0" OF THE ADMINISTRATIVE PLAN OF THE WEST MONROE HOUSING AUTHORITY, WITH REGARD TO EMERGENCY APPLICATIONS FOR THE ELDERLY, THE HANDICAPPED, AND THE DISABLED.
There was a motion by Mr. Bennett, seconded by Mrs. Pearson and unanimously carried to adopt a resolution of 1-Year and 5-Year Plans to be submitted to HUD.

A special meeting of the Board of Commissioners, Hospital Service District No. 1 of the Parish of Ouachita, State of Louisiana was called to order at 11:15 a.m. in The Wellness Center, 3215 Cypress Street, West Monroe, Louisiana, by Violet Liner, Chairman of the Board of Commissioners.

REMARKS FROM THE CHAIRMAN OF THE DISTRICT
Ms. Liner reported that the engagement of Luffey Huffman & Monroe has been approved by the Office of Legislative Auditor and that the financial statement deadline is now June 30th.
Mr. Landry, attorney for the Hospital Service District #1, has advised that the posting of a bond in favor of the Louisiana Department of Justice under the Physical Fitness Services Act in relation to The Wellness Center will not be necessary. An exemption letter was received from the Department of Justice.
REPORT ON RECENT ELECTION OF WARD FIVE HEALTHCARE FOUNDATION DIRECTOR BY OUACHITA PARISH POLICE JURY
Ms. Liner noted that a letter was received from the Ouachita Parish Police Jury announcing the appointment of Mr. Danny Van to the Ward Five Healthcare Foundation’s Board of Directors.
AMENDMENT OF THE AGENDA
Mr. Hall moved, Mr. Nelson seconded, and the Board unanimously approved amending the agenda to move the following agenda item before Executive Session:
“Approval of sale by Glenwood Resolution Authority, Inc.
of stock in Lincoln Health System, Inc. to same”
APPROVAL OF SALE BY GLENWOOD RESOLUTION AUTHORITY, INC. OF STOCK IN LINCOLN HEALTH SYSTEM, INC. TO SAME
After due discussion concerning the sale by Glenwood Resolution Authority of stock in Lincoln Health System, Inc. to same, the following resolution was offered by Mr. Nelson, seconded by Mr. Hall, and unanimously carried:
WHEREAS, Section 2.D(3) of the Lease Termination Agreement dated January 31, 2007 between the District and Glenwood Resolution Authority, Inc. ("GRA"), a nonprofit corporation (formerly named Glenwood Regional Medical Center) requires that GRA shall not enter into any transaction involving more than $100,000 unless the District otherwise consents;
WHEREAS, Section 7.21 of the Asset Purchase Agreement dated July 20, 2006 among the District, GRA, Glenwood Health Services, Inc., and IASIS Glenwood Regional Medical Center L.P. ("IASIS") requires that, if the ownership interest of GRA in Lincoln Health System, Inc. is transferred to a third party following the closing for consideration in excess of $1,500,000, such excess amount shall be paid to IASIS within five days following the consummation of such transaction;
WHEREAS, GRA, in consultation with IASIS, has negotiated the sale of GRA's ownership interest in Lincoln Health System, Inc. for the sum of $3,200,000, of which GRA will then pay $1,700,000 to IASIS in accordance with the Asset Purchase Agreement;
RESOLVED, that the District approves, ratifies, and consents to the sale by GRA of its ownership interest in Lincoln Health System, Inc. for the amount of $3,200,000 to Lincoln Health System, Inc. on the terms and conditions negotiated between GRA and Lincoln Health System, Inc. and the payment of $1,700,000 of such proceeds to IASIS in accordance with the Asset Purchase Agreement.
EXECUTIVE SESSION FOR THE PURPOSES OF STRATEGIC PLANNING, INCLUDING DISCUSSION OF THE IASIS TRANSACTION AND RELATED MATTERS, FUTURE OPERATION OF THE WELLNESS CENTER, IMPLEMENTATION OF THE LEASE TERMINATION AGREEMENT, AND OVERSIGHT OF WARD FIVE HEALTHCARE FOUNDATION
A motion was made by Mr. Nelson, seconded by Mr. Hall and unanimously approved for the Board of Commissioners to go into executive session for the purpose of strategic planning, including discussion
of the IASIS transaction and related matters, future operation of The Wellness Center, implementation of the lease termination agreement, and oversight of Ward Five Healthcare Foundation.
All present were allowed to remain.
A motion was made by Mr. Hall seconded by Mr. Nelson and unanimously approved for the Board of Commissioners to go back into regular session.
ADJOURNMENT
With no further business to discuss, Ms. Liner declared the meeting adjourned.

PUBLIC NOTICE
The Ouachita Parish School Board met in special session on Tuesday, March 27, 2007, at twelve (12:00) o’clock noon at its regular meeting place, the Ouachita Parish School Board office.
The following members were present: Jack White, Scott Robinson, A. R. Sims, Jerry R. Hicks, John Russell, Carey Walker and Phillip Odom. There were none absent.
Special Projects and Safety Coordinator Kenneth Slusher led the Pledge to the Flag.
Board member Phillip Odom gave the Invocation.
On motion by A. R. Sims, seconded by Jerry Hicks, the Board unanimously approved the Minutes of March 13, 2007 as written.
Upon motion by Phillip Odom, seconded by A. R. Sims, the Board approved the revised Agenda as presented. Motion carried unanimously.
On motion by Jerry Hicks, seconded by A. R. Sims, the Board approved the revised Personnel Agenda as presented including the changes since mailout. Motion carried unanimously.
Communication Specialist Aline Smead introduced Christy LeCroix, Public Relations with CenturyTel. Ms. LeCroix announced that CenturyTel is donating $3500 to the Art Quest Program to help defray the cost of printing art lesson plan books, which will be given to all teachers submitting poetry written by their students in a recent poetry contest. To be published are lesson plan books containing all twelve lesson plans for first, third and fifth grades and how the art program relates to the curriculum. The CenturyTel sponsors are also agreeing to publish poetry booklets containing 497 poems all submitted by students in addition to publishing booklets that include 197 short stories. CenturyTel was acknowledged for their continued support of Ouachita Parish schools.
Mr. Christian Creed of Creed & Creed Law Firm was recognized for his generous book donation to each library in all Ouachita Parish middle and high schools. The book is based on the trials and triumphs of black quarterbacks and is entitled, “Third and a Mile,” authored by William C. Rhoden, a “New York Times” best-seller author. During a book signing, two NFL greats and local products, James Harris and Doug Williams autographed copies of the book. Each donated book contains autographs of these famous players. The Board expressed their gratitude to Mr. Creed for his thoughtful contribution and continued support of our school system.
John Russell and Special Projects and Safety Coordinator Ken Slusher asked the Board for their consideration after presenting their re-negotiations for the purchase of land to construct a “new” Richwood Junior High School. Whereby, John Russell moved, seconded by Scott Robinson, that the Board authorize the purchase of the proposed Richwood Junior High School property containing twenty-three (23) acres at a cost of $15,000.00 per acre as presented, and that the cost be funded by means of the General Fund, further stipulating that three (3) acres of available property adjacent to the proposed site be considered in a timely manner if the bond issue is passed. Reverend Russell further moved, that this action be with the understanding that all transactions (fees) in connection with this land purchase is contingent upon the reimbursement of the General Fund once the passage of the bond issue election is approved by the voters in July 2007.
The following roll call vote was recorded: Jack White, yes; John Russell, yes; Carey Walker, yes; Jerry Hicks, no; Phillip Odom, yes; Scott Robinson, yes; and A. R. Sims, yes. Motion carried, with six (6) “yes” votes and one (1) “no” vote. Mr. Hicks clarified his “no” vote with the following statement: “Is not against the project out there, but against voting one-half million dollars out of the General Fund on the bases that we may pass the tax or we may not.”
Special Projects and Safety Coordinator Kenneth Slusher presented a proposal to purchase a house and lot next door to the house and two lots owned by the Board on Riggs & Cryer. Whereby, Scott Robinson moved, seconded by Jerry Hicks, that the Board purchase said property and house on Cryer Street in West Monroe for the purchase price of $40,000.00 with the intent of offering the house up for sale for removal. Motion carried unanimously.
On motion by Jerry Hicks, seconded by A. R. Sims, the Board granted permission to bid the purchase of parish wide musical instruments [Bid No. 35-07] and the paving and roofing of the Aviation & Military Museum [Bid No. 36-07]- (the OPSB is the fiscal agent for the museum). Motion carried unanimously.
The next Board meeting is set for April 17, 2007 at 12:00 noon.
There being no further business to be brought before the Board, Mr. Sims moved, seconded by Phillip Odom, that the meeting adjourn. Motion carried unanimously.

MINUTES OF THE REGULAR MEETING OF THE WEST MONROE CITY COUNCIL
APRIL 10, 2007
There was a Regular Meeting of the West Monroe City Council held on Tuesday, April 10, 2007, at 7:00 p.m. in the Council Chambers of the West Monroe City Hall.
Those present were Mayor Dave Norris, Aldermen L. O. Yeager, Alice Pearson, Fred Ragland and James Bennett. Absent: James Brian
The invocation was offered by Dr. Jerry Edmondson.
Major Scott Olvey, MNC-1C7, presented the Mayor and Board of Aldermen with a flag which had been flown over the Headquarters of the Multi-National Corps in Irag during Operation Iraq Freedom. This flag was in appreciation for the prayers and support of the American troops.
There was a motion by Mrs. Pearson, seconded by Mr. Bennett and unanimously carried to approve the minutes of the March 13, 2007 Regular Council Meeting.
There was a motion by Mrs. Pearson, seconded by Mr. Ragland and unanimously carried to approve payment of the March 2007 billings subject to audit by the Finance Committee.
There was a motion by Mr. Yeager, seconded by Mrs. Pearson and unanimously carried to approve an ordinance to amend Sections 12-7012, et seq of the Code of Ordinances (update of provisions regarding securing of buildings). Ordinance 3649:

There was a motion by Mr. Yeager, seconded by Mrs. Pearson and unanimously carried to approve an ordinance to revoke certain unused and unneeded utility servitudes affecting West Monroe Commercial Park Subdivision Unit 2 and the resubdivision of Lot 28AUnit 2, West Monroe Commercial Park Subdivision (Dean L. Hart, owner). Ordinance 3651:
AN ORDINANCE REVOKING PORTIONS OF UNUTILIZED AND UNNEEDED UTILITY SERVITUDES IN THE CITY OF WEST MONROE, LOUISIANA, BEING A PORTION OF THOSE UTILITY SERVITUDES EARLIER DEDICATED TO THE CITY IN WEST MONROE IN THE FILING OF WEST MONROE COMMERCIAL PARK SUBDIVISION UNIT 2, AND IN THE FILING OF THE RESUBDIVISION OF LOT 28A-UNIT 2, WEST MONROE COMMERCIAL PARK SUBDIVISION, AND FURTHER PROVIDING WITH RESPECT THERETO.
There was a motion by Mrs. Pearson, seconded by Mr. Yeager and unanimously carried to approve an ordinance to approve execution of a Cooperative Endeavor Agreement with the LA Department of Economic Development. (grant relating to North Central LA marketing) Ordinance 3652:
AN ORDINANCE AUTHORIZING THE MAYOR TO ENTER INTO A COOPERATIVE ENDEAVOR AGREEMENT WITH THE LOUISIANA DEPARTMENT OF ECONOMIC DEVELOPMENT RELATING TO MARKETING FOR THE NORTH CENTRAL LOUISIANA I-20 CORRIDOR REGION, AND OTHERWISE PROVIDING WITH RESPECT THERETO.
There was a motion by Mrs. Pearson, seconded by Mr. Yeager and unanimously carried to authorize the City Clerk to advertise for bids for small animal pens for the Ike Hamilton Pavilion.
There was a motion by Mrs. Pearson, seconded by Mr. Yeager and unanimously carried to authorize the City Clerk to advertise for bids for traffic signal improvements for the Downing Pines Road Interchange.
There was a motion by Mr. Yeager, seconded by Mrs. Pearson and unanimously carried to authorize the City Clerk to advertise for bids for permanent signage for the Downing Pines Road Interchange.
Bids were received for the Sparta Reuse Demonstration Project-Part D. There was a motion by Mrs. Pearson, seconded by Mr. Yeager and unanimously carried to reject all bids.
Bids were received on 3-27-07 for the City Court Building Expansion. There was a motion by Mr. Bennett, seconded by Mrs. Pearson and unanimously carried to award the bid to Don Leach Construction ($454,000.00).
There was a motion by Mr. Yeager, seconded by Mr. Bennett and unanimously carried to allow opportunity for public comment.
There being no further business, there was a motion by Mr. Yeager, seconded by Mr. Bennett and unanimously carried to adjourn.

SEPARATE SEALED BIDS FOR STREET IMPROVEMENTS IN THE BAWCOMVILLE COMMUNITY, OUACHITA PARISH, LOUISIANA, FY 2006 LCDBG Program to be funded by the Louisiana Community Development Block Grant Program for Fiscal Year 2006 will be received by the Ouachita Parish Police Jury, (hereinafter referred to as OWNER) at the office of the Secretary-Treasurer in the Parish Courthouse Building, 300 St. John Street, Monroe, Louisiana, until ten (10:00) o'clock A.M. CDST, Tuesday, May 22 , 2007. Sealed bids will be publicly opened and read aloud at that time. Any bids received after closing time will be returned to the bidder unopened.
All bids must be submitted on the proper bid proposal forms. The contractor must display his contractor's license number prominently on the outside of the envelope. Each bid shall be sealed and plainly marked as "Bid for Street Improvements in the Bawcomville Community, Ouachita Parish, Louisiana, FY 2006 LCDBG Program."
The Contract Documents including complete plans and specifications may be examined and procured at the office of the Engineer, Harrison and Associates, Inc., Consulting Engineers, 200 Washington Street, 2nd Floor, Monroe, Louisiana. A refundable deposit of $50.00 shall be required for each set of Contract Documents.
If the bidder is a corporation, a corporate resolution authorizing a representative of the corporation to sign the bid must accompany the bid.
Each bidder must deposit with his/her bid, security in the amount, form, and subject to the conditions provided in the Information for Bidders. Sureties used for obtaining bonds must appear as acceptable on the U.S. Department of Treasury Circular 570.
For contractor information, this project is classified as municipal and public works construction.
No bidder may withdraw his/her bid within thirty (30) days after the actual date of the opening thereof.
Attention to bidders is particularly called to the requirements as to conditions of employment to be observed and minimum wage rates to be paid under the Contract, Section 3, Segregated Facilities, Section 109, Executive Order 11246, and all applicable laws and regulations of the Federal Government and State of Louisiana and bonding and insurance requirements.
