CALL TO ORDER: A special meeting of the Board of Commissioners, Hospital Service District, No. 1, Parish of Ouachita, State of Louisiana, Glenwood Regional Medical Center, was called to order at 7:35 a.m. by Violet Liner, Chairman, in the Board Room on the Fifth Floor of the hospital. ENGAGEMENT OF BOND COUNSEL: Ms. Liner noted that she had been speaking with Meredith Hathorn, Bond Counseler with Foley & Judell, LLP concerning the Hospital Revenue Bonds. In so doing, it had been brought to her attention that the District had not formally engaged Ms. Hathorn's services.
The following resolution was offered by Bill Nelson and seconded by Mr. Mulhearn:
A resolution providing for the employment of bond counsel in connection with strategic planning as it relates to Glenwood Regional Medical Center.
BE IT RESOLVED by the Board of Commissioners of Hospital Service District No. 1 of the Parish of Ouachita, State of Louisiana (the "District"):

SECTION 1. That there is a need for the employment of bond counsel in connection with strategic planning as it relates to Glenwood Regional Medical Center, including any defeasance or refunding of the outstanding Bonds of the District, and accordingly, Foley & Judell, L.L.P., New Orleans, Louisiana, be, and they are, hereby employed as bond counsel to the District to do and to perform comprehensive and coordinate professional work with respect thereto. The fee to be paid bond counsel shall be an amount not exceeding the Attorney General's then current Bond Counsel Fee Schedule and other guidelines for comprehensive, legal and coordinate professional work, together with reimbursement of out-of-pocket expenses incurred, said fee to be subject to the Attorney General's written approval of said employment.
SECTION 2. That this resolution shall take effect immediately.
This resolution having been submitted to a vote, the vote thereon was as follows:

And the resolution was declared adopted on this, the 17th day of April, 2006.
V. MOTION TO GO INTO EXECUTIVE SESSION:
A motion was made by Mr. Spencer seconded by Mr. Hall and unanimously approved for the Board of Commissioners to go into executive session for the purpose of strategic planning including financial matters.
All present were allowed to stay.
A motion was made by Mr. Nelson, seconded by Mr. Hall and unanimously approved for the Board of Commissioners to go back into regular session.

Notice is hereby given that, in accordance with L.R.S. 3:1609 and LAC 7:XV.314(A), the Louisiana Department of Agriculture & Forestry, Louisiana Boll Weevil Eradication Commission, has established a boll weevil eradication zone, the Louisiana Eradication Zone, consisting of all the territory within the state of Louisiana.
Notice is further given that all producers of commercial cotton in Louisiana are required to participate in the boll weevil eradication program, including cost sharing, in accordance with the Boll Weevil Eradication Law and Rules and Regulations. This includes, but is not limited to, reporting of cotton acreage and paying of assessments by July 15 of each crop year, and destruction of cotton plants and stalks by December 31 of each crop year. Only Contractors licensed according to Contractor's Licensing Law, R.S. 37:2151-2163, shall be considered if his bid is greater than $50,000.00 and shall show his license number on exterior of Bid Envelope and above his signature or the signature of his duly authorized representative.

Sealed bids for a Document Imaging System for the Business Office for the Ouachita Parish Public Library will be received in the Director's Office of the Ouachita Parish Public Library, Main Branch, 1800 Stubbs Avenue, Monroe, La., until 4 p.m., Monday, May 22, 2006 Any manufacturer's names, trade names, catalogue or model numbers, or other similar material identifying a particular product, of a particular grade of product, are used for descriptive purposes, to establish the standard of quality, design, and performance. However, any substitution or departure proposed must be clearly noted and described. Otherwise, it will be assumed that the Bidder proposed to supply the items as specifically described in these Specifications. It is understood and agreed that the Bidder, if awarded the subject contract, will protect, defend and hold harmless the Library, its members, agents, and employees, from any claims, suits, or demands. This includes demand for payment for the use of any trade secret that may enter into the design, manufacture, or use of the item or services contracted for. Including any material, device, article or process the use or which is substantially important to the proper and most effective use of the items or services contracted for. Bidder further agrees to indemnify in full the Library, its members, employees and agents for any amounts which they may be required to expend in the defense of such claims, suits of demands, or in settlement thereof, or in satisfaction of any judgement, award or decree resulting therefrom. Homegrown systems will not be considered. All items in the specifications must be addressed in some way either in the positive or the negative. Specific requirements: Document system: * * The library requires an integrated electronic document management system that can manage at least 5000 documents per month or other digitally captured documents. * * The system must be capable of using high-speed or other standard scanners regardless of manufacturer. * * The system must be able to verify that data has been scanned and indexed properly without individual user input. * The imaging system must utilize a Windows 2000 or higher platform at the server (already on site) and client level. * It must be capable of using either Microsoft DE or MS-SQL. Images must be capable of being saved in a variety of standard formats such as PDF, JPG, GIF, TIFF, or BMP. * Must be capable of converting scanned images to full text documents or other formats. Indexing must be automatic. * The system must be capable of scheduling tasks to be done unattended at a later time such as after hours. * Must have built in migration capabilities to transfer data from one volume to another or to CD-ROM, DVD, or off site. * Must have an advanced audit trail to log activities by function, date, and user. * The system should have an integrated FTP manager, which allows automatic FTP of completed documents as the job is in progress. * Data backup must be allowed, built into the system and schedulable to multiple types such as tape, CD-ROM, DVD, or any other storage unit. Retrieval system: * Zoom, rotate, and magnification capabilities. * Must allow for annotation without changing the underlying image. To include a "sticky note" text annotation. * Ability to edit annotations via user login without changing the underlying image. * Be able to restrict or enable access to documents and annotations in document level security enabled fields. * Support automatic launching of documents embedded in an email. * Retrieval available via the web at any workstation on the network without a special client. * Allow for global searching across multiple projects. * Allow auto updating of folders. The price quoted should include installation, setup, and training for any staff requiring it. Maintenance and upgrade fees, if applicable, must be included with the proposal, but will not be purchased until all warranties expire. The successful vendor must be able to provide local support and live technical support from software developers during normal library working hours (8:00 a.m. - 5:00 p.m.), CST. Evaluation Criteria: (In no particular order) 1. The vendor must have local (within 2 hours driving time) technical support and have an agreement with the image software company. 2. The vendor must be a channel partner or licensed representative of the image software proposed. 3. The vendor must provide the names of other customers they maintain on the imaging software. 4. The ability to provide on-site training. 5. The scalability, expandability and upgradability of the proposed software. 6. A live demonstration of the system proposed must be completed at the library before consideration of a system. 7. The vendor's experience with imaging systems. 8. The Fox Run MHP Water System is currently in violation of the maximum contaminant level (MCL) for total trihalomethanes (TTHMs) as set forth by the State [Part XII of the Louisiana State Sanitary Code (LAC 51:XII)] and the Federal Primary Drinking Water Regulations (40 CFR Part 141). The United States Environmental Protection Agency (EPA) and the Louisiana Department of Health and Hospitals (LDHH) sets drinking water standards and requires the disinfection of drinking water. Where disinfection is used in the treatment of drinking water, disinfectants combine with naturally occurring organic and inorganic matter present in water to form chemicals called disinfection byproducts (DBPs). EPA and LDHH set standards for controlling the levels of disinfectants and DBPs in drinking water, including trihalomethanes (THMs). Some people who drink water containing THMs in excess of the MCL over many years may experience problems with their liver, kidneys, or central nervous system, and may have an increased risk of getting cancer. In December 1998, EPA set enforceable drinking water standards for TTHMs at 80 parts per billion (ppb) to reduce the risk of cancer or other adverse health effects. Compliance with the TTHM standard for public water systems serving less than 10,000 individuals initially became effective and enforceable on January 1, 2004. Compliance with the TTHMs standard is determined by calculating a running annual average (RAA) of quarterly TTHMs sample results. Compliance calculations performed during the first calendar quarter of 2006 show that the system‚s current TTHMs RAA is 193 ppb; thus, the system is currently in violation of the TTHMs standard. Please share this information with all the other people who drink this water, especially those who may not have received this notice directly (for example, people in apartments, nursing homes, schools, and businesses). You can do this by posting this notice in a public place or distributing copies by hand or mail. This is not an emergency. If it had been, you would have been notified immediately. EPA and LDHH do not consider this violation to have any serious adverse health effects on human health as a result of short-term exposure; however, continued long term exposure to TTHMs levels above the standard (e.g., 20 years of exposure) has the potential to have serious adverse effects on human health.
MINUTES OF THE SPECIAL MEETING OF THE WEST MONROE CITY COUNCIL MAY 3, 2006 There was a Special Meeting of the West Monroe City Council held on Wednesday, May 3, 2006, at 9:00 a.m. in the Council Chambers of the West Monroe City Hall. Those present were Mayor Dave Norris, Aldermen L. O. Yeager, James Bennett and Alice Pearson. Absent: Fred Ragland and James Brian. There was a motion by Mr. Bennett, seconded by Mr. Yeager and unanimously carried to approve a resolution ordering and calling a special election to be held in the City of West Monroe, State of Louisiana, to authorize the renewal of a sales and use tax therein; making application to the State Bond Commission in connection therewith; and providing for other matter in connection therewith. Resolution 548: A RESOLUTION ORDERING AND CALLING A SPECIAL ELECTION TO BE HELD IN THE CITY OF WEST MONROE, STATE OF LOUISIANA, TO AUTHORIZE THE RENEWAL OF A SALES AND USE TAX THEREIN; MAKING APPLICATION TO THE STATE BOND COMMISSION IN CONNECTION THEREWITH; AND PROVIDING FOR OTHER MATTERS IN CONNECTION THEREWITH. Bids were received for Dissolved Air Flotation Units (waste water treatment plant). They were as follows: NAME BID Poly Disc Inc. 67,000 per unit - 2 units There was a motion by Mrs. Pearson, seconded by Mr. Yeager and unanimously carried to accept the bid if it meets the specifications.
