Ouachita EOC officials are compiling a list of local Hurricane Gustav damages to submit to the federal government in hopes of receiving a federal disaster declaration for Ouachita Parish residents who have experienced any primary structural or water damages as a result of Hurricane Gustav have two ways to report this information.

1. Visit the following website: http://www.ohsep.org
Click on Individual Damage Assessment Form. You can download if you wish to fax, or send electronically by filling the form out the online form.

2. Call (318) 322-4335 between 7 a.m. and 7 p.m. to report damages. There is only one phone line so, if you receive a busy signal, keep trying until 7 p.m. each day. Additional lines may soon be installed.

At this point, Ouachita Parish is not included in the federal disaster declaration. Governor Bobby Jindal has made a request to President Bush to amend the federal disaster declaration to include at Louisiana parishes. With the assistance of local, state and federal officials, we hope that by showing the extent of our local damages, it will help us earn this designation and clear the way for federal relief assistance.
